Photo Release *

I hereby give permission for my child(ren) to be photographed during the 2024 CTTA Mt Calvary Kids Summer Camp. I understand the photos will be used to keep a journal of activities, for promotional purposes including flyers, brochures, newspaper, website, and social media. I understand that although my child’s photograph may be used, his or her identity will not be disclosed. I understand there will be no compensation and that all photos are the property of Central Texas Tennis Association (CTTA) and/or affiliates.

In consideration of myself or my child(ren), being allowed to participate in the program sponsored by CTTA, I, the undersigned parent/guardian, binding heirs, executors, administrators, estate and assigns, do hereby release and agree not to hold liable CTTA or Mt Calvary Missionary Baptist Church, and/or its officers, board, agents, employees or volunteers; from any and all actions causes of action, claims, demands, costs or damages as a result of property damages or personal injuries or death sustained by me or my said child or his/her or my property, arising from or resulting from an act or omission, negligent or otherwise, of CTTA or Mt Calvary Missionary Baptist Church, programs, officers, board, agents, employees, or volunteers while participating in the 2024 Mt Calvary Kids Summer Camp activities or while traveling to or from the location at which such activities will be conducted. In the event of any medical emergency, I authorize and consent for CTTA Mt Calvary Kids Summer Camp to act on behalf for medical care deemed necessary for the participant by Certified Emergency Personnel (i.e. EMT, First Responder, and/or Physician).
